1. What is Remote Monitoring with HealthSnap?
Remote monitoring allows your provider to monitor your health data (ex. Blood Pressure) in relation to the status of your diagnosis, right from the comfort of your home.
HealthSnap is an app that makes it easy to monitor and share your weight, blood pressure, and glucose, alongside your lifestyle, with your provider anytime, anywhere.
Examples of Health Data: Blood Pressure, Resting Heart Rate, Glucose
Weight Examples of Lifestyle Data: Sleep, Nutrition, Physical Activity, Stress
2. How does the program work?
If you are a candidate, your provider will prescribe and send a Remote Monitoring protocol specific to your diagnosis to HealthSnap and they will verify your coverage with your insurance company.*
If covered, your personal HealthSnap “Angel” will reach out, enroll you in the program and at no cost to you, provide you with either a blood pressure cuff or weight scale your provider prescribed for you.
Once received, your personal HealthSnap “Angel” will contact you, set you up and have you transmitting your health data from home for your provider to monitor in real time.
